Lecture

Summary of the incident

The aircraft crew encountered technical malfunctions during the approach for landing:

  • deterioration of the pilot's condition

  • cabin pressurization problems

  • failure of the landing gear and flaps to extend

  • loss of pressure in the brake hydraulic system

Several go-arounds were performed, and the landing was ultimately carried out on an unpaved runway in emergency mode.

What was done correctly

The crew:

  • Continuously reported parameters: altitude, heading, distance, and system status

  • Decided to make repeated approaches in order to stabilize the situation

  • Handed control over to the co-pilot when their condition worsened

  • Performed a manual charge of the hydraulic system and prepared for a flapless landing

  • Remained calm and kept altitude under control despite the system failures

Air traffic controllers:

  • Provided accurate data on headings, altitude, and distance

  • Checked on the technical condition of the aircraft and the crew

  • Offered an alternative landing strip (unpaved)

  • Coordinated the actions of the rescue services

  • Gave recommendations on operating the systems (feathering, brakes, flaps)

Consultant:

  • Assisted the crew with technical advice on the hydraulic system, landing gear struts, and hatches

What can be considered errors or risks

The crew:

  • Several times failed to extend the landing gear and flaps in a timely manner — it may have been better to switch to manual control sooner

  • There were signs of conflict between the pilots ("give me control") — this is critical in an emergency situation

  • The distress signal was not switched on immediately, despite the deteriorating condition

Air traffic controllers:

  • At certain points repeated commands without checking the crew's response

  • Did not always respond promptly to emotional signals ("I'm feeling worse and worse")

Overall assessment

Despite the technical failures and the stressful environment, the crew and air traffic controllers acted in a coordinated manner. The landing was completed successfully, taking all constraints into account. The key roles were played by:

  • competent communication

  • readiness for manual control

  • support from the consultant
